This is for you to RENT a 9 Plus breast pump (to buy please go back to the product page and choose the full priced item).
Please follow these instructions
1. Fill in these forms - please take a screenshot of the submission pages and send them to me (note there are 2 forms):
* https://forms.office.com/r/racGMgCnQi - note the form says the payments will be collected by 'EzyPay', however, I am currently trialling a new company called 'GoCardless' (18/8/24), there are no fees to customers via this company, but by filling in the 2 forms you authorise GoCardless and myself to debit the amounts as per the Rental Agreement form for the breast pump and any other products taken.
* https://pay.gocardless.com/BRT0003B5F73N3F - enter the bank account you wish to be used for the rental payments to be taken from (at the moment Australian bank account debits are the only options - please let me know if this is an issue)
2. Pay for the deposit by adding this item to your cart and complete the order just like a normal online purchase.

Pick up near Morley is usually available every day, including weekends (but again, if you need the pump urgently it is best to contact me first to double check)
